Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Financial and Insurance
We provide quality dental care at an affordable cost. We accept the following payment options: Cash, check, Visa, MasterCard, AMEX, and Discover. We also offer financing through CareCredit. CareCredit is a credit card exclusively for healthcare services. CareCredit offers low monthly payment plans for healthcare procedures not commonly covered by insurance. For more information regarding financing and payment options, please contact the office.
A discount of 10% or 8% is offered to patients who pay in full at time of service. A 10% discount will be given to patients who pay with cash or check and an 8% discount with credit card payments (excludes CareCredit, Delta Dental policy holders and Superior Dental policy holders).
If you have dental insurance, our goal is to maximize you insurance benefits. Our practice accepts most insurance plans. As a courtesy to our patients, we will submit your dental insurance claims. The amount of your insurance company’s payment is determined by the level of coverage purchased. You will be responsible for any remaining balance not covered by insurance. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
